How to insert iStent glaucoma implant Key points: 1. Tilt head away from you. (If head was taped for the cataract surgery, have technician break the tape). 2. Tilt microscope nasally. 3. Set magnification and focus 4. Place viscoelastic on cornea 5. Place gonioprism on eye to visualize angle (Gonioprism: Swan-Jacob, Ritch, Khaw, etc.) 6. Inspect iStent, place index finger on iStent inserter button 7. Place iStent into the main wound, near to the pupil, without having placed the gonioprism yet. This is so the iStent will be at the pupil, near the angle, and will not risk touching the cornea or go beneath the iris. 8. Place gonioprism on eye 9. Set magnification and focus and visualize trabecular meshwork 10. Engage trabecular meshwork at 15 degrees approach 11. Drive iStent into trabecular meshwork using wrist (blood reflux is normal) 12. Once inserted, release iStent by pressing button 13. Tap iStent into place using inserter 14. Remove gonioprism first, then iStent, so can visualize the inserter being removed 15. Move head back into ortho position 16. Remove viscoelastic 17. Hydrate wounds
