When you come to cancer detection, there are certain things that we need to consider. If we look at the literature, a breast implant can obscure the view of the breast tissue in a mammogram, theoretically increasing the death rate from breast cancer due to late detection of the cancer. Breast implants can be placed either below the muscle or above the muscle; the literature suggests that if the implant is placed below the muscle, mammograms are much easier and it does not prevent early detection of breast cancer because the implant is not obscuring the breast tissue itself. The same thing cannot be said about implants placed above the muscle, where there can be issues with routine mammograms and the early detection of breast cancer. Nevertheless, there are actually no studies suggesting that implants decrease the survival rate of patients who develop breast cancer after breast augmentation surgery. If you have breast implants, you should still get regular screening mammograms like somebody who does not have implants. The indications for a mammogram are the same. However, if you have implants, you need to tell the technologist before starting the mammogram. You will get a diagnostic mammogram rather than a screening mammogram. The X-rays used in mammograms cannot go through silicone or saline implants well enough to show the breast tissue under them. This means that the part of the breast tissue covered by the implant won’t be seen on the mammogram. Woman with implants have four extra pictures done (two on each breast), as well as the four standard pictures taken during a screening mammogram. In these extra pictures, called implant displacement (ID) views, the implant is pushed back against the chest wall and the breast is pulled forward over it. This allows better imaging of the front part of each breast. Implant displacement views are more difficult to do and can be uncomfortable in women who have had a capsule contracture. They’re easier in women whose implants are placed underneath (behind) the chest muscles. So, in essence: 1. Breast implants do not cause cancer. 2. Depending on their location, implants can hamper the detection of breast cancer with mammograms, but no studies have shown that breast implants decrease the survival rate of breast cancer patients when we compare patients with and without implants.
