This case underwent operative e hysteroscopy for treatment of uterine septum. The uploaded video lasts 10 minutes showing the procedure and the tragedy at end. What went wrong ? 1) cutting the septum went in different planes going up and down several times leading to irregular cut and variable thickness fundus. 20 Apparently surgeon lost orientation several times and direction of cut went steep up or down. 3) The reference point landmark was lost all through procedure ( Osteum ) 4) It was obvious near end that myometrium became very thin and transparent fundus but surgeon kept of cutting till fundal perforation happened. 5) Surgeon did not recognize perforation and did not inform patient upon possible sequale. Patient got pregnant few months later by aid of IVF. At 20 weeks gestation suffered acute abdomen and adnitted to emergency unit. Ultrasound showed very rare view! The pregnancy sac extruded from a big perforation at fundus rupture. Patient was operated immediately , repair of uterus and 4 units of blood transfusion given.
