They are supplied by 1 or 2 arteries which are supplying blood to them. So if you are able to locate those blood vessels and close the, some of the early stages of piles, simple close the blood vessels there is no need for any major surgeries like cutting, removing, wounds, post-operative pain and complications are avoided by just closing the supply. Ligation is closing the vessels. So haemorrhoidal arterial ligation or HAL is a very good procedure for early stages of piles. Where we have been doing for more than 10 years with excellent results. Only thing is how to detect the arteries which are supplying the haemorrhoids. To detect that there is a gadget called as doppler. So there is a doppler embedded procotoscopy used to detect in the OT sitting where the results are there and through that inside only we will operate. So there are no outside wounds, there is no removal of the tissue. Only it is simple closed. If it is at a later stage, 3rd or 4th degree piles what we call, where the piles are coming out, there we add one more procedure called as recto anal repair, to HAL procedure. There the excess of tissue is pushed back using a different type of stich. So in these procedures because there are no wounds, patient is able to go back to his work within a day or two and able to sit comfortably post operative bleeding is very much reduced but the thing is it requires a little expertise to learn the process. Once the process is mastered, it gives excellent results. As far as the complications or problems after the procedure are not very risky procedure. In the beginning stages of learning there are very small small issues. Once the procedures are learned properly and done properly, selected properly, it gives excellent results.
