Jovanovic. These are the key points of the procedure 1. Preoperative Planning • Patient Evaluation: Assess bone volume using CBCT to determine the extent of resorption and plan augmentation. establish healthy periodontist and soft tissues • Surgical Plan: Decide on the amount of vertical and horizontal augmentation needed for future implant placement. 2. Surgical Approach • Anesthesia & Incision: Administer local anesthesia with sedation if necessary. Make a full-thickness crestal incision with releasing incisions for adequate flap mobilization. • Flap Elevation: Carefully elevate the mucoperiosteal flap to expose the atrophic ridge while preserving blood supply and minimizing trauma. 3. Site Preparation & Bone Grafting • Cortical Perforations: Create small perforations in the cortical bone to enhance vascularization and graft integration. • Bone Graft Placement: harvest and Pack particulate autogenous bone mixed with xenograft to provide stability and osteoconductive properties. And maximizing regeneration. 4. TR-PTFE Membrane Placement • Membrane Selection: Use a non-resorbable TR-PTFE membrane to create space and prevent soft tissue ingrowth. • Fixation: Secure the membrane using titanium pins and screws to maintain stability over the grafted site. • Tension-Free Closure: Mobilize the flap through periosteal release to achieve primary closure without tension. 5. Postoperative Care & Healing • Antibiotics & Pain Management: Prescribe antibiotics (e.g., amoxicillin or Zpack) and NSAIDs for pain control. • Suture Removal: Typically done after 2–3 weeks, ensuring primary closure is maintained. • Membrane Removal: Since TR-PTFE is non-resorbable, it must be removed after 9+ months, allowing full graft maturation before implant placement. 6. Implant Placement (Second Stage) • CBCT Evaluation: Assess the augmented ridge volume after 9 months. • Flap Reopening: Remove the TR-PTFE membrane and evaluate new bone formation. • Implant Insertion: Carefully place implants with proper angulation and stability for long-term success. This approach ensures predictable bone regeneration, creating the necessary vertical and horizontal volume for successful implant placement.
