Echinocandins don't poison the fungus; they sabotage its architecture. While they are the "safe" darlings of the ICU, using them blindly is a recipe for disaster. In this lecture, we explain why these drugs are surgical for Candida but useless for Cryptococcus and Molds, and why "starting an echinocandin" isn't a substitute for a clinical diagnosis. Part 3 of our "Antifungal Drug Classes" series focuses on the β-(1,3)-D-glucan synthase inhibitors. We move beyond basic pharmacology to discuss DM/NEET SS-level concepts: the biology of the fungal cell wall, the "Eagle Effect," and the rising threat of FKS gene mutations in the ICU. A septic ICU patient with persistent candidemia is on Micafungin. The species is identified as C. glabrata. Despite 5 days of therapy, blood cultures remain positive. Do you increase the dose, or do you look for something else? Hint: Antifungals cannot sterilize hardware. Answer in the comments. "Which Echinocandin is your 'go-to' in the ICU? (A) Caspofungin - the classic, (B) Micafungin - the reliable, or (C) Anidulafungin - the renal-friendly? Let's discuss why you chose it below."
