INTRODUCTION Intravenous regional anesthesia is a procedure in which analgesia is produced in part of a limb by intravenously injecting local anesthetic while occluding circulation to the affected limb. The method can be used in either the upper or lower extremity to produce excellent anesthesia for hand, wrist, ankle, and foot surgery of relatively short ( under 90 minutes) duration. It is less well suited to more proximal procedures (of the elbow and of the knee). Advantages include reliability, ease of performance, safety, rapid onset, controllable duration of action, good muscular relaxation, and a rapid recovery. Disadvantages include the potential for toxic reactions and the use of a vascular-occluding tourniquet that limits the safe duration of the block and patient tolerance of tourniquet discomfort.
