Diabetes mellitus is a long-term metabolic disorder in which the body is unable to properly regulate blood glucose levels. This happens because of either a lack of insulin, which is the hormone responsible for moving glucose into cells, or because the body’s cells become resistant to the effects of insulin. As a result, sugar builds up in the bloodstream, leading to persistent hyperglycemia. Over time, this imbalance affects the metabolism of carbohydrates, fats, and proteins, and if not controlled, it can cause damage to many organs of the body. There are different types of diabetes. Type 1 diabetes usually develops in childhood or adolescence and is caused by an autoimmune destruction of the insulin-producing cells in the pancreas, leading to absolute insulin deficiency. Type 2 diabetes, which is much more common, generally develops in adulthood and is mainly due to insulin resistance combined with inadequate insulin secretion. Other forms include gestational diabetes, which appears during pregnancy, and some rare genetic or secondary causes such as MODY (maturity-onset diabetes of the young), LADA (latent autoimmune diabetes in adults), or diabetes due to pancreatic disease or certain medications. The typical symptoms of diabetes include excessive thirst (polydipsia), frequent urination (polyuria), increased hunger (polyphagia), unexplained weight loss, fatigue, blurred vision, recurrent infections, and slow wound healing. Diagnosis is usually made through blood tests such as fasting plasma glucose, oral glucose tolerance test, HbA1c, or random blood glucose levels when accompanied by symptoms. If left untreated or poorly managed, diabetes can lead to serious complications. In the short term, these include diabetic ketoacidosis (DKA) and hyperosmolar hyperglycemic state (HHS), both of which are medical emergencies. Over the long term, diabetes can damage small blood vessels (causing retinopathy, nephropathy, and neuropathy) and large blood vessels (leading to heart attack, stroke, and peripheral artery disease). Management of diabetes requires a combination of lifestyle changes and medical treatment. Healthy eating, regular exercise, and maintaining a healthy body weight are essential. Medications may include oral drugs that improve insulin sensitivity or secretion, as well as insulin injections for those who need them. Newer treatments, such as GLP-1 receptor agonists and SGLT2 inhibitors, provide additional options for blood sugar control and cardiovascular protection.
