The Malaise: A disease infects the island kingdom, transforming its citizens into grotesque monsters.  The King: The king, in a desperate attempt to stop the spread, orders mass arrests and executions, even of those merely suspected of infection.  The Alchemist/Collector: The king also orders the alchemist (later revealed to be the Collector) to find a cure, leading to questionable experiments and the creation of the Panacea, a cure that requires cells.  The Beheaded: You are the Beheaded, a homunculus of the king, trapped in a time loop, forced to repeatedly fight through the infected island, seeking a way to escape and understand the events that led to the kingdom's downfall.  The Time Keeper: The Time Keeper, a mysterious figure, seems to be responsible for the time loop, attempting to give the island a chance to find a solution to the Malaise.  The Collector's Plot: The Collector, in his pursuit of the Panacea, steals cells from The Beheaded, revealing his true intentions to become an all-powerful god, leading to a final confrontation.  The True Ending: After defeating the Collector, The Beheaded realizes the island is doomed and that the Malaise may have spread beyond the island.
