Key to safe dissection of parotid lesions are first locating Facial Nerve trunk at Tragal Cartilage pointer or otherwise tracing one of major branches and dissecting in a retrograde manner from there. Intra-parotid branching pattern of facial nerve is variable with cross innervations being the most among buccal and zygomatic branches and least for marginal mandibular branch. Marginal mandibular branch is therefore the one to be focussed and must always tried to be secured in parotid surgeries to avoid facial deformities post-operatively. Superficial Partial Parotidectomy as offered in this case gives better outcome compared to ECD (extra capsular dissection) as in later approach, recurrence is observed ( about 2% in a decade).
