White mature cataracts can pose problems with completing a continuous tear capsulorhexsis (CCC), due to the capsules being more fragile and leakage of liquefied cortical material may occur in hypermature cataracts. The capsulorhexis tears tend to extend to the periphery because of high intracapsular pressures associated with the liquefied cortical material. If the capsule rips in two directions the "Argentian Flag" sign will be produced when using Tryptan Blue staining of the capsule. Little's maneuver can sometimes save the errant rhexsis but if the tear goes to the equator, it is best in my experience to convert to a can-opener type capsulotomy. Phacoemulsification can be performed easily with a can-opener capsulotomy as well as in the bag implantation of the IOL. The nucleus can then be dismantled either supra- or intra-capsularly.
