This film was created for the JAC and FirstDatabank sponsored 'Improving Patient Safety' category of the 2012 Isle of Wight NHS Awards held in November 2012. The project aimed to reduce loss of mobility through amputation and neuropathic ulceration in patients with diabetes on the Isle of Wight, by using specialist Podiatrists to carry out tailored lower limb vascular and neurological assessments. This was delivered in primary care and the resulting risk status allowed targeted early intervention, enhanced self management and efficiency savings whilst delivering much improved outcomes for patients with diabetes. This work has lead to fewer amputations and fewer admissions. It has promoted joint working between primary and secondary care as well as increasing patient involvement. With each amputation estimated to cost £68,000 this has led to massive savings and improved the quality of life for these patients. For more information visit www.iow.nhs.uk/awards.
