Castrate Resistant, Non-Metastatic Prostate Cancer (M0 state) | Prostate Cancer Staging Guide

PCRI’s executive director, Mark Scholz, MD, gives an overview of the Low-Royal stage of prostate cancer (that is, cancer that has developed a resistance to hormonal treatment but has not yet metastasized beyond the pelvic lymph nodes) and the new second-generation hormonal agents used to treat it. 0:31 This video is concerned with men in the “Low-Royal” stage of prostate cancer. Low-Royal is defined as a situation in which a man who is being treated with hormone therapy for Indigo stage prostate cancer (that is to say relapsed prostate cancer) begins to experience a rising PSA despite low testosterone levels. With Low-Royal there are no detectable metastases beyond possible spread into the pelvic lymph nodes (compared to Basic-Royal and High-Royal which have 5 or less, or 6 or more metastases outside of the pelvic lymph nodes, respectively). 1:23 The most significant feature of the Low-Royal stage is resistance to hormonal treatment. Since it is the most effective method for preventing the spread of the disease, this is a serious development. Without treatment, men will develop bone metastases in an average of 18 months after developing hormonal resistance. 2:04 Some doctors will tell patients that a rising PSA on low testosterone is nothing to be worried about, but it is actually a sign that things are changing quickly and something must be done. 2:33 In the past, there were no treatments for men at this stage; however, there are now second-generation hormone therapies such as Xtandi, Erleada, and the forthcoming Darolutamide that can delay the onset of metastases by two years with relatively few side effects beyond those already caused by first-generation hormone therapies. 2:56 Rigorous studies have demonstrated that second-generation agents delay metastases by an average of two years. Even more, these studies were conducted on men with PSAs that had already risen to the 8-10 range. Resistance to hormonal treatment can be easily be detected when a PSA is less than 1.0, and men who get treatment before their PSAs rise to the 8-10 range do even better than the men in the studies. It is important, then, for men with rising PSAs on hormonal treatment to start treatment as soon as possible 4:01 The common terminology doctors use to describe a Low-Royal situation is “M0” where “M” means metastases and “0” means zero visible metastases.
