⚕️ FREE MSRA PODCAST – Labial Adhesions in Children 🎧 A high-yield deep dive into a common but often overlooked paediatric condition – ideal for your MSRA revision or clinical practice. 🧠 Key Learning Points 📌 Definition • Labial adhesions = fusion of the labia minora (inner lips), usually in prepubertal girls (3 months to 3 years). • Often resolves spontaneously by puberty. • ⚠️ Not the same as an imperforate hymen (which is congenital and blocks the vaginal opening). 📌 Etiology & Risk Factors • Likely caused by local irritation or inflammation. • Contributing factors: – Low oestrogen levels – Recurrent UTIs – Poor hygiene – Skin irritation or trauma – Underlying skin conditions 📌 Pathophysiology • Inflammation → sticky labial surfaces → fusion at the midline • Thin, semi-translucent adhesions may partially or completely block the vaginal opening 📌 Differential Diagnosis • Lichen sclerosis • Urethral prolapse • Congenital anomalies • Vaginal foreign body • Imperforate hymen • Vaginal stenosis 📌 Epidemiology • Common in prepubertal girls • Usually benign and self-limiting 📌 Clinical Presentation • Thin adhesion seen on examination • May present with: – Recurrent UTIs – Urinary frequency or retention – Discomfort or pain – Urine pooling after micturition 📌 Diagnosis • Clinical – based on visual examination • Further investigations only if uncertain or complications suspected (e.g. biopsy for lichen sclerosis) 📌 Management 1. Conservative – if asymptomatic – Reassure and advise good hygiene 2. Medical – if symptomatic – Topical oestrogen cream applied to the adhesion 3.
