It's an autoinflammatory condition which is similar to psoriasis or lichen planus or rheumatoid arthritis or other conditions where your immune system attacks certain parts of the body. So in Vitiligo the melanocytes get damaged. In the early stages the melanocytes stop producing melanin and with certain medications and phototherapy they can be stimulated to produce melanin again. Whereas in certain other cases the melanocytes completely have lost the ability to produce melanin. In these cases we need to do grafting one of the types of grafting done in Vitiligo is punch grafting. Punch grafting is favored for its ease it can be done by anybody doesn't require much training. Any dermatologist can do it but it requires an operating theater setup. So you need to have a sterile operating room you need to have instruments like 1mm punches or 1.2 mm punches to do the grafting and it's preferred over bony areas because bony areas don't respond well to other types of grafting especially if you are getting on the knuckles or on the elbows or on the ankle area. So those bony areas we prefer to do Punch grafting. Punch grafting is a slightly painful procedure. So it's done with local anesthesia. Once the procedure is done we'll be giving the patient certain painkillers for two to three days. If they take those painkillers they'll be absolutely comfortable. So the dressing is changed usually on the 5th to 7th Day depending on which area we are operating and once the punch sits into the skin completely then we start doing treatment called phototherapy where the pigment spreads from the area where we have put the punch it spreads to the surrounding area. So it has a higher success rate than most other grafting procedures because it contains entire thickness of the skin it contains epidermis as well as dermis and both are important in producing the pigmentations. Most of the other grafting procedures have only epidermis or only melanocytes.
