Simple method of detecting outward movement of DLT during surgery using a neonatal resucitation bag which is attached to the DLT on going to one lung ventilation, instead of opening to atmosphere. . On going to one lung ventilation do not open lumen to atmosphere pressure. Instead attach a collection bag, such as the neonatal resucitation bag as a monitor for leaks around the distal cuff. Air in distal cuff minimal to prevent evulsion into trachea, and so early leak can be measured by collecting air. Easier correction of outward DLT displacement after surgery has started. Testing for leaks in lung simplifed. Low cost, simple solution.
