Excessive Traction During Shoulder Dystocia: When Pulling Too Hard Causes Permanent Injury When a baby becomes stuck during delivery and a doctor pulls too hard, serious and permanent injuries can occur. This situation often happens during shoulder dystocia, when the baby’s head delivers but one shoulder becomes trapped behind the mother’s pubic bone. It is a medical emergency that requires calm, trained maneuvers — not panic. Doctors are trained to use specific techniques to safely resolve shoulder dystocia, including: • The McRoberts maneuver • Suprapubic pressure • Rotational maneuvers • Controlled delivery techniques However, when those maneuvers fail — or when a provider panics — excessive traction may be applied. This can happen with: • Forceps • Vacuum extraction • Manual pulling Excessive traction can damage the brachial plexus nerves, which control movement and sensation in the shoulder, arm, and hand. These injuries may result in: • Erb’s palsy • A “floppy” arm at birth • Weakness or limited range of motion • Permanent nerve damage in severe cases While some brachial plexus injuries resolve over time, others are permanent and may require surgery, therapy, or lifelong treatment. These injuries are often preventable when proper techniques are used and excessive force is avoided.
