A paraovarian cyst is a benign fluid-filled sac located near, but not attached to, the ovary, often discovered incidentally during ultrasound exams. On ultrasound, it appears as a well-defined, anechoic structure with smooth, thin walls, positioned about 1 centimeter from the adjacent, otherwise normal ovary. Color Doppler imaging typically shows no internal vascularity, consistent with its benign nature. Key differential diagnoses include ovarian cysts, which are attached to the ovary and may contain internal echoes; hydrosalpinx, which appears as a fluid-filled, tubular structure; peritoneal inclusion cysts, characterized by irregular shapes and loculations; and endometriomas, which present with homogenous low-level echoes. The prognosis for a small paraovarian cyst is excellent, with a low risk of complications such as torsion or rupture. Management is usually conservative, involving periodic ultrasound monitoring. Surgical intervention is rare and reserved for symptomatic cases or those showing significant changes. Patient education is important, particularly in advising them to report any new symptoms like pelvic pain. #ultrasound #ovary #cyst
