Lateral Window Approach - Bone Augmentation of the Maxillary Sinus

Lateral window sinus grafting also called direct sinus lifting - is a surgical procedure that is performed to increase bone volume in a pneumatized maxillary sinus. When posterior maxillary teeth are extracted - the sinus pneumatizes over time reducing vertical alveolar bone dimensions. This restricts implant placement due to inadequate bone volume. This surgical procedure augments the sinus allowing the surgeon to place implants in adequate bone volume. Case report: 45 yr male - missing #16 - #17 (lost 20 years earlier) - severe pneumatized maxillary sinus - direct approach - lateral window created - augmented with a mix of 50:50 cortico-cancellous bone and xenograft (porcine origin). PRF membranes placed into and over the sinus window with a collagen membrane. Allowed 6months of remodelling time - implant placed - primary stability achieved - healing abutment placed - split thickness flap of the tissue performed to roll the connective tissue - helps to augment the buccal tissue thickness - wait for 4 months - closed tray impressions - restored with a screwmentable (custom titanium base connection with a zirconia crown).
