We begin with the breakdown of dietary fats, including triglycerides, phospholipids, and cholesterol esters, starting in the small intestine. The lecture details how bile salts, secreted from the gallbladder, emulsify large lipid droplets into smaller micelles, increasing surface area for pancreatic lipase activity. We then explain the role of pancreatic lipase and colipase in hydrolyzing triglycerides into free fatty acids and monoglycerides, and the contribution of cholesterol esterase and phospholipase A2 in breaking down other lipid types. Next, we focus on lipid absorption in the enterocytes. Micelles transport lipid digestion products to the brush border membrane, where they diffuse into enterocytes. Inside the cell, lipids are re-esterified into triglycerides, phospholipids, and cholesterol esters, which are then packaged into chylomicrons along with apolipoproteins. These chylomicrons are secreted into the lymphatic system via lacteals before entering the systemic circulation. The lecture also highlights the importance of bile salt recycling via the enterohepatic circulation and the clinical consequences of malabsorption syndromes.
