This video was part of the Video Loop during the SAGES 2020 Virtual Meeting. Topic: Colorectal Use of a manually operated combined needle knife / injection catheter for colonic ESD eliminates the need for instrument exchanges. Neil Mitra, MD, Xiaohong Yan, PhD, Vesna Cekic, RN, HMC Shantha Kumara, PhD, Richard L Whelan, MD. Lenox Hill Hospital/Northwell Health ESD is the gold standard for removal of large sessile adenomas and superficial cancers. Establishing and maintaining a mucosal “lift” by injecting saline or other solution into the submucosa is a critical requirement. Once established, lifts are refreshed via either: 1) sclerotherapy catheter insertion and injection (post removal of knife) or 2) injection via a needle knife that contains a thin catheter (no instrument exchange needed). This video demonstrates ESD done with a new combined needle knife/catheter that utilizes manual injections (no machine pump). The ability to refresh the lift and to immediately cut tissue facilitates ESD and shortens cases.
