Dr. Ebraheim’s educational animated video describes the important facts concerning the Teres Minor muscle. The teres minor is a narrow muscle which originates from the posterior lateral surface of the scapula and inserts into the greater tuberosity of the humerus. The teres minor muscle is innervated by the posterior branch of the axillary nerve. It is believed to be the same nerve that gives innervation to the lateral part of the shoulder. Important anatomical space Quadrangular space Boundaries: •Teres minor (superior) •Teres major (inferior) •Long head triceps (medial) •Surgical neck humerus (lateral) Triangular space •Teres major •Teres minor •Long head triceps •Contains the circumflex scapular artery One of the best way to see the teres minor muscle is on a sagittal section of an MRI. The infraspinatus is the main external rotator of the shoulder with the arm to the side (adducted). The teres minor muscle is the main external rotator of the shoulder with the arm abducted. Clinical examination Horn blower’s test •Test is used to determine the strength of the teres minor muscle. •The patient and examiner are both standing and the patient’s arm is elevated to 90 degrees •The patient’s elbow is then flexed to 90 degrees and the patient is asked to laterally rotate the shoulder. •Weakness and/or pian against resistance signals a positive test. Why is the teres minor important? •It is a very good test exam question ( in-training exam, board exam, self-assessment) •Innervation is unexpected, it comes from the posterior branch of the axillary nerve. •It is important in posterior approach to the shoulder joint •It is important when it is atrophied: atrophy of the muscle can be idiopathic or due to quadrangular space impingement.
