We have a unique case where we have a patient coming in and he's complaining of gynecomastia. But his case is unique in that the skin excess is actually more of an issue than the glandular tissue. So when I look at it, I want to address the most important thing first, which is the skin excess and also the very enlarged areola. The point being is I can't do both at the same time. I can't reduce the size of the areola and then remove the gland underneath it. Or what will happen is the areola could have some problems associated with it in terms of wound healing. So what we're going to do today is; we're going to do a donut lift, effectively. Where we're going to reduce the size of the areola, and I am going to reduce a little bit of the glandular tissue on the bottom half. But I need to keep a blood supply so that it heals well.
