Congenital Aqueductal Stenosis with Hydrocephalus 8 Months first born male child out of Non-Consangious marriage presented to us with increasing head size and irritability. Child Had History Of premature delivery at 8 months and neonatnal jaundice On Examination Patient Had large head with head circumference of 46cms with bulging fontanelle and dialted scalp veins and sun set sign being present. Patient Was Investigated With MRI Brain Which Showed dilated lateral and third ventricle with normal sized fourth ventricle and aqueductal stenosis. Imaging Features Were Suggestive Of congenital Aqueductal stenosis With Hydrocephalus With PVL. patient was planned for Endoscopic third ventriculostomy after taking consent and telling the chances of failure of ETV in such young infant. patient underwent right precoronal burrhole and ETV post-op patient had no neurological deficits and hydrocephalus reduced as evident by decreasing head size to 44cms, depressed fontanelle and resolved sunset sign.
