The average medical student graduates with $180,000 in debt. While tuition makes up the biggest chunk, students are further burdened by a number of costly standardized tests. All medical students must take the United States Medical Licensing Exam, the USMLE, which is made of 3 steps. Each step consists of a lengthy written exam, but step 2 also includes a Clinical Skills, or CS, component where students are judged based on simulated encounters with patient actors. Evaluating the clinical skills of future doctors is a good thing! But we don’t need a national exam to do it. More than 90% of US medical schools already run their own clinical skills exams similar to Step 2 CS. These schools provide students detailed feedback to help them become better doctors. What do students get after taking Step 2 CS? 98% of US students get little more than one word: Pass. Each year, 20 thousand students pay the $1,275 exam fee to take Step 2 CS. Altogether, students spend more than $36 million per year, with these costs disproportionately burdening students from disadvantaged backgrounds. And this doesn’t even include the cost of travel and lodging to get to one of only 5 cities where Step 2 CS is offered. And yet the numbers do not lie, The National Board of Medical Examiners, the organization that develops and manages the USMLE has seen their revenue double since Step 2 CS was rolled out. And although they are a Non-profit organization, their CEO earns $1 million. Ironically, $1 million is exactly how much it costs to catch just one student who fails the exam. Step 2 CS is a poor value. It’s expensive. It has no proven safety benefit. And nearly every medical school already runs their own more useful exam. We are a national movement to End Step 2 CS.
