DMEK for a failed DSAEK

A patient presented with reduced vision 4 years following previous successful DSAEK surgery. The reduced vision was secondary to graft failure and secondary corneal decompensation. The original DSAEK button was removed and replaced with just Descemet's membrane and endothelium in a DMEK procedure. This surgical video demonstrates the technique. It also highlights the importance of marking the donor graft which is particularly helpful in very loose scrolls. In this case, the DM scroll was very loose and the graft was initially upside down. The graft was flipped to the correct orientation and the patient did extremely well after the surgery.
