ERCP Stone Extraction (Mechanical Lithotripsy) and Plastic Stent Placement

A 67 years gentleman presented at SQUARE HOSPITAL LTD Dhaka Bangladesh with Obstructive jaundice, Choledocholithiasis. FINDINGS : PAPILLA: Looks normal. Bile flow seen. PANCREATOGRAM: Not attempted. CHOLANGIOGRAM: The CBD, CHD and the intrahepatic ducts are hugely dilated. Large number of filling defects consistence with stones are seen in the CBD and CHD. Papillotomy is done by conventional papillotome followed by large balloon sphincteroplasty. Multiple stones are extracted by billiary basket sweeping. Large stone is grasped by mechanical lithotripter and crashed. Crashed stones are extracted out by repeated balloon sweeping. A 10Fr X 5cm pigtail plastic stent is implanted into the bile duct with establishment of free flow of bile. COMMENTS: Choledocholithiasis (Please see the description also). THERAPEUTIC INTERVENTION: ERCP with papillotomy, balloon sphincteroplasty, stone extraction and billiary stenting. ADVICE: Removal of stent and check ERCP after one month.
