In this surgery, a retinal reattachment procedure for pseudophakic RD, I am about to do a fluid air exchange (FAX) for pneumohydraulic reattachment of the retina after an adequate vitrectomy. You can see the air bubble coming in and I am active as well as passively sucking the fluid out through the cutter or the flute cannula. During the fluid air exchange, as you can see in the video, the media becomes hazy. And the reason for that is a common problem that we encounter in such cases - condensation of water droplets and air bubbles on the intraocular lens resulting in fogging of the posterior surface of the lens. This problem can be tackled by injecting viscoelastic between the air bubble and the lens as seen here in the video. The viscoelastic effectively coats the IOL and prevents further fogging.
