Understanding complex anatomy and tumour spread in lateral skull base surgery for parotid glands Lateral skull base surgery for parotid tumors requires a thorough understanding of the intricate anatomy involving bone, muscle, vessels, and nerves from the zygoma to the digastric, and the potential for extensive tumor spread in this region. b. Role of preoperative imaging and planning Comprehensive preoperative imaging, particularly MRI, and meticulous planning, including 3D reconstructions, are essential for accurate staging, defining resectable structures, and anticipating potential complications. c. Surgical techniques and considerations for resecting structures Resection strategies must be tailored to the tumor's involvement of specific structures like the mandible, condyle, external auditory canal, mastoid, and facial nerve, often requiring en bloc removal with careful attention to preserving uninvolved tissues. d. Strategies for avoiding complications and ensuring safe resection Avoiding complications involves meticulous anatomical understanding, careful cranial nerve evaluation (especially after level II neck dissection), assessing internal carotid artery involvement with balloon test occlusion if needed, and respecting tumor biology to guide the extent of resection. e. Importance of reconstruction and facial reanimation Planning for reconstruction, often with pedicled flaps like PMMC or anterolateral thigh, should be integrated with the initial incision to ensure adequate vascularity, and facial reanimation should be considered during the same surgical setting to improve patient outcomes.
