📉When I see a #partialkneereplacement candidate in the office, I discuss the many benefits of partial knee compared with #totalkneereplacement 👍🏼 These benefits include quicker surgery, less blood loss, returning home the same day, saving all the ligaments of the knee (including the ACL), which allows for a more natural feeling knee, and and higher patient satisfaction. 👎 The main downside I discuss with these patients is the lower long-term survival rate. However, one of the biggest reviews of the survival between TKA and UKA (unicompartmental knee arthroplasty = partial knee replacement), published in the Lancet medical journal in 2019 reviews data from the early 2000s going back up to 25 years. This data, which places 15-25 year survival of UKA at 80% and dropping to 70% by 25 years. The TKA groups reviewed in this study maintain survival in the range of 92+% at 20 years. ☝️However, comparison of more recent designs from the American Joint Replacement Registry shows no significant difference in survival at 6.5 years. While the TKA group appears to be about ready to split and demonstrate significance in improved survivorship after this time point, only time will tell if this trajectory remains. With improvements in popular designs such as a second peg on the Oxford femoral component (which has demonstrated survival benefits in other national registries at 10 years in the 94% range), and the use of robotics (which show survival benefits at 5 years), it is possible that the improved survival of TKA may not be what it used to be. 👇 Do you tell your partial knee candidates that their surgery won’t last as long as a total knee?
