Atrial fibrillation with rapid ventricular response (AF with RVR) is defined by an irregularly irregular rhythm with a ventricular rate greater than 100 bpm—and it can significantly reduce cardiac output by losing atrial kick and shortening ventricular filling time. This video reviews two common rate-control medications used for AF with RVR: Cardizem (diltiazem) and verapamil. These are Class IV calcium channel blockers used to slow the ventricular rate—not necessarily to “fix” the atrial fibrillation immediately. What you’ll learn ✅ Why AFib can be dangerous: decreased stroke volume and clot risk ✅ Why we often focus on rate control first (especially when rhythm conversion isn’t appropriate) ✅ Practical use differences: ▪️Cardizem is used more often in the hospital (commonly stored refrigerated) ▪️Verapamil is used more often in the field/prehospital setting Dosing: Cardizem (diltiazem): ▪️0.25 mg/kg IV over 2–3 minutes ▪️May repeat 0.35 mg/kg ▪️Then infusion 5–15 mg/hr Verapamil: ▪️5 mg IV (emphasis on a slow push; “5 mg over 5 minutes” is highlighted) Safety Because calcium channel blockers can cause hypotension and bradycardia, the key is slow administration—time the push and reassess the patient as the rate comes down. 💬 Comment below: In your setting, which do you see more often for AF with RVR—Cardizem drip or verapamil push—and why?
