This case presents a very early intrauterine pregnancy with a small gestational sac and an embryo barely visible on transvaginal ultrasound. A large subchorionic hematoma is noted as a hypoechoic collection between the chorion and uterine wall, increasing the risk of threatened abortion. Additionally, a corpus luteum cyst is seen in the ovary, showing mild vascularity but without a clear "ring of fire" sign. Serial ultrasounds are recommended to monitor fetal viability, hematoma resolution, and pregnancy progression. Supportive management, including close observation and possible progesterone supplementation, may be considered.
