Today we are going to discuss IUI, which stands for intrauterine insemination. In this video, we will cover what IUI is, its indications, the process, its drawbacks, and its success rate. Intrauterine insemination (IUI) is a procedure that has been performed for a long time. Many patients come to us after having multiple failed IUI attempts and often believe it is a mild procedure that comes before IVF. Some patients request IUI even when IVF is advisable. It’s important to understand that while IUI is simpler than IVF, it is not a substitute for it. If IVF is indicated, IUI should not be performed as an alternative. Indications for IUI: IUI can be done in cases of mild male factor infertility, where there is a slight issue with sperm count, motility, or volume. It is also indicated for sexual dysfunction, unexplained infertility, or if the cervix is not conducive for sperm to move up. We make sure to clearly explain the indications for IUI to our patients to ensure it is done only in appropriate cases. Process of IUI: The success rate of IUI is similar to that of a natural cycle because the pregnancy occurs through natural mechanisms. Here’s a breakdown of the process: Day 2 of Periods: The patient comes in, and we administer oral drugs or injectables to stimulate egg production, aiming for 1-2 eggs. Day 11-13: We give an injection to induce ovulation. 36 Hours Post Injection: IUI is performed. We take a semen sample from the husband, prepare it in the lab by concentrating the sperm in a small amount of media, and then insert this concentrated sample into the uterus near the fallopian tubes. This preparation ensures that only the sperm, free from other substances in semen, are used. The IUI process involves inserting a catheter through which the sperm are released into the uterus. The entire process takes about 14-15 days, requiring 4-5 visits for ovulation scans and the actual procedure. Success Rate and Limitations: The success rate of IUI is about 15-20%, sometimes as low as 10-15%, especially in older patients or those with low AMH levels. IUI is less effective in cases of unexplained infertility or poor egg quality. It’s crucial for patients to understand these success rates to manage their expectations. It’s important to check that the fallopian tubes are patent before proceeding with IUI. If the tubes are blocked, IUI will not be successful, leading to unnecessary costs and disappointment. Post-IUI Care: Patients are advised to continue with their normal activities post-IUI. Bed rest or avoiding routine activities is not necessary. Some patients may experience mild abdominal pain due to ovulation or cramping from the procedure, which is normal. Number of IUI Cycles: Typically, we recommend up to 2-3 cycles of IUI. If these cycles fail, it is unlikely that IUI will be successful in subsequent attempts, and other options should be considered. Pain and Comfort: IUI is generally a painless procedure performed in an outpatient setting. Proper instructions and compliance play a significant role in the success of the procedure.
