Red reflex illumination setting on zeiss microscopes can be achieved by turning the illumination knob into position 1. This setting provides optimum red reflex by reducing scleral glare and is helpful in dense cataracts. In first case of near total cataract, we could perform capsulorhexis without staining the capsule as red reflex illumination provides excellent contrast. In the second case of mature cataract, even after staining the anterior capsule with trypan blue, red reflex illumination provides better contrast not just for rhexis, but also when hydrodissection is done. Fluid wave can be appreciated in this setting. The third case is that of dense cataract with long anterior zonules or LAZ, which become prominent in position 1 of illumination setting and their shearing can be appreciated better. Fluid wave during hydrodissection confirms that there is no zonular weakness even though anterior zonules have sheared during rhexis.
