Surgical approach would involve either a median sternotomy approach or right thoracotomy to perform sleeve resection of the left proximal bronchus with lymph node sampling and frozen section analysis to confirm negative margins. Complication rates are low. For completely resected tumors with negative margins, adjuvant radiation is not recommended. If the tumor were only debulked or there was nodal disease or positive margins, adjuvant radiation at 54 Gray would normally be recommended. Following treatment, surveillance should be performed with CT scans every 6 months for the first several years and bronchoscopy at 6 months and then annually thereafter.
