Today we’re talking about the FT3 to FT4 ratio. I received a lot of questions about this and wanted to explain a little further. What this ratio indicates is the conversion of T4 to T3 at the tissue level and potentially the thyroid gland compensation. Keep in mind that most of the T3 in the body is not made in the thyroid gland, it’s made in the kidneys, liver and peripheral tissues that have the ability to convert T4 to T3, primarily by deiodinase 2. What this can help us understand is how well the tissues are converting the T4 that’s in the bloodstream into T3. I talk a lot about the cell stress response. Cells get to determine what happens to T4. Just because there’s plenty of T4 in the bloodstream and plenty of T4, and potentially T3, getting to the hypothalamus doesn’t mean that the rest of the tissues are functioning in a high metabolic state. Different cells and tissues have the ability to self-regulate. Using the ratio will help determine how well the body is converting T4 to T3, which is what we really want to know because T4 can quickly normalize the TSH and we think we’re good, but what causes hypothyroid signs and symptoms is low T3 inside the cells. If your doctor never looks at your T3 or FT3 level, how do you know if T4 is actually being converted to T3 optimally?
