This video demonstrates a important salvage technique for managing intra renal pelvi ureteric junction obstruction. In select cases, it can salvage a renal moiety by establishing a communication between the dependent part of dilated intra renal collecting system and ureter. The key steps of the procedure include, dissection of UPJ, dissection of the lower dependent collecting system within the lower pole of kidney, segmental lower polar nephrectomy with or without warm ischemia, everting hemostatic sutures between the collecting system and lower pole cortex and finally a stented, tension free, dependent and watertight anastomosis between ureter and collecting system.
