This presentation by Prof. Chin-Su Liu from Taipei Veterans General Hospital (TVGH) reviews the role of liver transplantation (LT) in the treatment of hepatocellular carcinoma (HCC), drawing on the experience of TVGH. Both deceased donor liver transplantation (DDLT) and living donor liver transplantation (LDLT) have been performed at TVGH, with a total of 846 cases between 2004 and 2023. While early studies showed poor outcomes for LT in HCC, the introduction of the Milan criteria improved results by selecting patients with smaller tumors. Later, the UCSF criteria expanded the selection criteria. Expanded criteria for LDLT, such as the 5-5-500 rule in Japan and the Kyoto criteria, have also been developed. The presentation also explores the role of ALBI score in assessing patients with very early stage HCC and reviews factors to reconsider when evaluating treatment options for early-stage HCC.
