We call our technique Istanbul Varicocelectomy technique. It has 4 components. 1. We remove external spermatic veins completely, sometimes longer than 20 cm. 2. We use operation microscope to see all vascular structures in a magnified and detailed way. 3. We use Microvascular Doppler intraoperatively, to find main artery faster and easier. And we find subbranches of main artery which will diminish the blood supply of testis if not protected properly. Unfortunately, it is not possible to find these arteries using only operation microscope. Because the microscope needs pulsation of the vessels which is not possible to see in each case. 4. The last but the most important step of Istanbul Varicocelectomy technique is removal of internal spermatic veins in long segments. This may be a crucial step to prevent the recurrence of varicocele surgery. In standard varicocele operation we tie and cut enlarged veins. So we left behind the testicular part of the varicose vein which is already large but it gets larger when we left it as a blind plug (the artery still fills the blood system of testis). In our method, we remove the varicose venous structures as long as possible. We disect them in very long segments. Our aim is to take out all the enlarged structures.
