MRI SHOULDER CARTILAGE DELAMINATION & FISSURE  Chondral abnormalities at the shoulder joint are a little bit harder to assess than they are in the knee because the thickness of the cartilage is much less. So the early chondral abnormalities, such as fissuring or delamination, are difficult to find in the shoulder, because the cartilage is so thin. In this case, it's unusual in that we're actually seeing it pretty well. So here we have normal cartilage over the humeral head, and as we get to the central portion, it thins out a bit, and then we see, A high signal cleft that goes through the cartilage, so here is the full thickness Chondral fissure. And then we see delamination or undercutting of the adjacent cartilage, so you can see that fluid is tracking underneath the adjacent cartilage, here is the overlying cartilage, here is the fluid tracking underneath it, and then again on this side you can see high signal fluid tracking between cartilage & bone, and this is the area of delamination. Early chondral abnormalities of the shoulder are difficult to see, such as fissuring or fibrillation or delamination. The most common thing we're going to see is actually just thinning of cartilage, but these very early changes of fibrillation or fissuring or delamination can be seen, although they're much less common.
