Cloudnine Hospitals, Patparganj, New Delhi Ectopic pregnancy is a pregnancy which is not in the uterus. It can be commonly in the fallopian tube. Less commonly in the ovary or in the abdomen. So ovarian ectopic, the entity itself is a very rare entity and the diagnosis here is of like paramount importance because it's not very easily diagnosed we need a high level of suspicion wherein on ultrasound we realize that the pregnancy is not seen in the tubes not seen in the uterus and then still the beta is rising. So then we suspect that it can be ovarian pregnancy on ultrasound. The radiologist can pick up a ring of fire appearance in the ovarian area when we see the betahCG values if the beta CG values are still low because as I told the diagnosis can be delayed in ectopic . So if the beta CG levels are high we straight away take the patient for surgery and when we do a tubal ectopic we sometimes remove the whole of the tube because still the patient is left with another tube and but when we are doing ovarian ectopic we have two ovaries but the hormone supply is very important and we do not want to sacrifice the ovary. So sometimes if it is a localized ectopic and the ovary has not ruptured it is not bleeding we go for a wedge reection of the ovary and we preserve the remaining ovarian tissue. In case the ovary has ruptured then we have to do a ectomy what's ectomy that's removal of the ovary and if the patient has come to us with ruptured in ectopic then there's a lot of hemoperitoneum. So we have to clear all the blood the clear the abdominal cavities remove that ovary then the patient has one ovary and the tubes left and then posttop. We have to make sure that all the lost blood is replenished with we give a proper antibiotic cover because we cannot afford any risk of infection in the ovarian tissue and then the patient is discharged on day three.
