Small pupils are one of the identified risk factors for a variety of intraoperative complications during cataract surgery. Many small pupil cases are managed successfully with iris hooks or stretching devices. However, these devices are associated with an additional operating time, and in some mild to moderate cases, the operation could be performed safely without them. Case description: This is a case of cataract surgery with small pupil performed on the right eye of an 88-year-old patient. The examination revealed that the patient has a pseudoexfoliation syndrome and the following refraction: RA: -1.50 -0.50 / 3 VA (decimal) = 0.4 LA: +0.5 -1.25 / 91 VA (decimal) = 0.4 We decided to remove the cataract without using iris hooks and to implant the new LuxSmart™ EDOF lens.
