Acrylic IOL exchange by Warren Hill, MD

This is an exchange of a single piece acrylic IOL for a 3-piece arcylic IOL. The single piece IOL had been in the capsular bag for six months. The lens capsule is first dissected from the anterior surface of the IOL with a 30-ga. needle on a syringe with a dispersive viscoelastic, such as Viscoat. After the initial dissection has been made in all quadrants, a blunt, flat spatula is used to separate the anterior and posterior portions of the more peripheral lens capsule by sweeping. This is most easily begun at the optic-haptic junction. A paracentesis in each quadrant is requited for complete access. The IOL is then gently mobilized and finally cut in two with MST IOL scissors. With the capsulorrhexis preserved, it is possible to place the new 3-piece acrylic IOL into the capsular bag.
