Giant retinal Tear | 2 step procedure ! This is a case of Giant Retinal Tear with a a rather posterior location of tear with PVR changes in the form of wrinkling and shortening of temporal retina. In this case we decided to use a two step approach in which we first settle the tear and retina , fill up PFCL to the brim and then after 360 degree endolaser we close the case. A second procedure is done after 2 weeks in which we exchange the PFCL with silicon oil. By doing this the retina gets time to settle at ease and the contraction tends to relax. If we put oil right away then at times such a case would start to contract and re detachment with tear edges retraction is quite common. In the first step Peripheral shaving and trimming of the anterior edge of the tear was also carried out. PFCL heavy liquid was injected to gradually unfold the tear and flatten the macula. After this 360 endolaser barrage was carried out. At this point the case was closed and patient was reoperated after 2 weeks. In the second step (2.08 seconds in the video clip) Air PFCL exchange was done taking care that the edges of the tear are kept dry throughout to avoid slippage.
