CataractCoach™ 2198: piggyback IOL with small surgical PI peripheral iridotomy

This patient had cataract surgery performed elsewhere which resulted in an unwanted refractive outcome with an IOL power that was too low. For an IOL power that is more than desired, resulting in a myopic surprise, we can perform keratorefractive surgery like LASIK or PRK to adjust the focusing. However, it is challenging to add significant dioptric power to the cornea with excimer laser ablations. We could also perform an IOL exchange for some cases like this, however here the IOL very adherent to the capsular bag and the prior anterior capsular opening is contracted and irregular. There is also some capsular fibrosis which is further locking in this single piece acrylic toric monofocal IOL. In this case, our best choice is to perform a piggyback IOL keeping in mind that 1.5 diopters at the IOL plane which be about the same as 1.0 diopter a the spectacle plane. For example, if the desired refraction at the spectacle plane is +2.0 diopters, then implanting a +3.0 diopter piggyback IOL should address that accurately. Since the haptic-optic junction of this three-piece silicone IOL is angulated 5 degrees, doing a peripheral iridotomy is not absolutely required, but in this case we perform one for added measure.
