A married jail employee is arrested after an inmate submits multiple requests to speak with investigators. What he eventually revealed about the woman who was supposed to be providing him professional services led to a felony investigation — and a story no one inside that facility saw coming. In this video, we break down the body camera footage, the inmate interview, the arrest, and the full investigation into a case that exposed a serious breach of trust inside a Michigan correctional facility. In January 2022, investigators at the Kent County Correctional Facility in Grand Rapids, Michigan sat down with an inmate housed in disciplinary segregation. He had submitted multiple request forms asking to speak with someone. He was afraid of retaliation. He didn't want more trouble. But what he described was something investigators could not ignore. The inmate had a documented mental health diagnosis. He was considered one of the most vulnerable individuals in the facility. The woman at the center of his claims was 43-year-old Theresa Lyn Simmons — a contracted mental health counselor hired to provide behavioral health services inside the jail. What investigators found in the inmate's cell corroborated his account. Handwritten letters containing graphic content were recovered. The letters referenced her personal life and acknowledged she could lose her license. The case was forwarded to the prosecutor's office. Charges were declined — twice. The inmate then filed a federal civil rights lawsuit. Months later, the prosecutor reversed course and authorized charges. On August 16, 2022, Simmons was arrested and charged with Criminal Sexual Conduct 2nd Degree involving a county inmate — a felony punishable by up to 15 years. She was transferred to a different county jail and her bond was set at $15,000. In March 2023, she entered a no contest plea. The original felony was dismissed. She was convicted of aggravated assault and providing a controlled substance to an inmate. Her sentence — $1,575 in fines. No jail time. She was terminated from her position.
