Ask an Endo Surgeon | What are the stages of endometriosis?

Dr. Madhu Bagaria explains each stage of endometriosis. Dr. Madhu Bagaria, MD, FACOG is board-certified in Obstetrics and Gynecology and Laparoscopic Surgery. As an expert in women’s and men’s health and sexual health, she has extensive experience treating pelvic pain conditions, abnormal uterine bleeding, and endometriosis. Dr. Madhu believes in listening to her patients, understanding their experiences and their individual needs, and working with them on a treatment plan that works best for their condition and lifestyle. At Pelvic Rehabilitation Medicine, our pelvic pain specialists provide a functional, rehab approach to pelvic pain. When you visit one of our offices, you spend an hour with your doctor reviewing in detail your medical history and symptoms. Then, we perform an internal exam (no speculum) to evaluate your nerves and muscles. Together, we'll discuss an individual treatment plan that gets to the root cause of your pain and helps you to feel better. The best part: you can begin treatment the same day! At PRM, our mission is to decrease the time patients are suffering from pelvic pain symptoms. Today I'm going to discuss about the stages of endometriosis. The endometriosis is staged according to the ASRM and has been diagnosed into four stages from stage 1 being minimal disease and stage 4 being more severe disease. One thing I want you guys to know that the severity of the pain does not correlate with the severity of disease, which we find when we go inside the abdomen in order to remove the endometriosis. What I mean by this is that people with a small lesion on the peritoneal lining can present with severe pain, and patients, we have seen patients with severe disease which may not have any pain. When we talk about the stages of endometriosis, it depends upon the location of the disease, the depth of the, the depth of the invasion, whether they are superficial or they are deeper implants, the presence of the lesions. In stage one, we find very small lesions, which could be on the surface lining of the peritoneum and they're very superficial lesions and they can easily be removed. Stage 2 is a moderate disease with few, few more, with more lesions, which are more extensive. They can be superficial disease or some deeper invasion lesions. It might involve small cysts in the ovaries. Stage 3 does involve chocolate cysts in the ovaries, which is basically the endometriomas in the ovary. You may have heard the word endometriomas, which is basically the cells similar to the lining, when they are invading on the surface of the ovaries, they can bleed inside and can form these chocolate cysts. And then stage 4 is more extensive disease when the whole posterior colony sac, which is the space between the rectum and the uterus, is obliterated. And can cause severe, it can be totally asymptomatic, as I said before, or it can cause symptoms as well.
