Continuing the discussion on Vital Pulp Therapy, I am sharing a video showing the treatment of a mandibular second molar with the diagnosis of irreversible pulpitis. You will see that a full pulpotomy was the treatment of choice in this case, with the surgical procedures brought to the level of the orifices. You will notice that, after removal of the roof, the pulp tissue in the chamber, although partly vital, appeared detached from the pulp chamber walls. I have histological evidence (not published yet) that this clinical feature is associated with severe inflammatory reactions, degenerative changes, and bacterial colonization. The pulpotomy level was brought to the canal orifices, where ideal conditions of the pulp wound were observed. The wounds were capped with a pre-mixed tricalcium silicate cement, and the re-entry after one year showed that the surgical sites had beautifully healed through the deposition of a mineralized tissue, with ideal clinical and radiographic conditions.
