Osteomyelitis, a bone infection, exhibits distinct morphological features that help in diagnosis. Two key pathological findings in chronic osteomyelitis are sequestrum and involucrum. Diagnostic Morphology of Osteomyelitis Sequestrum A necrotic, devascularized bone fragment that separates from healthy bone. Acts as a nidus for persistent infection, as bacteria within it are isolated from antibiotics. Appears as a dense, sclerotic fragment surrounded by a radiolucent rim on imaging. Involucrum A layer of new reactive bone that forms around the sequestrum. Represents the body's attempt to wall off the infection and promote healing. Seen as thickened periosteal bone on radiographs. Additional Features Cloaca – An opening in the involucrum allowing pus drainage. Granulation Tissue – Surrounds sequestrum, indicating chronic inflammation. Bone Marrow Changes – Edema, necrosis, and fibrosis in affected areas #Osteomyelitis
