We love white cataracts because it is a true miracle of modern medicine where we can take truly blind people and restore their vision to normal with a beautiful and elegant surgery. There are additional risks, particularly with the patients who have intumescent white cataracts, where the capsulorhexis can run-out. To prevent this, our guest surgeon has started using the spiral-out technique where the capsulorhexis starts small and then spirals out to the desired size. If there is loss of control and the rhexis starts to run-out, you can quickly pull centrally to complete the tear and then once the bag is decompressed it can be enlarged.
